455 Fine-Needle Aspiration, Ultrasound and Thyroidectomies in Patients with Thyroid Nodules Presenting to a District General Hospital – a Retrospective Audit

Abstract Aim To determine whether fine-needle aspiration, ultrasound and thyroidectomies were being performed appropriately in patients with thyroid nodules, to compare the percentage of malignant cases for each histological classification national standards. Method Patients undergoing thyroidectomy Northwest Anglia Trust (East England) between October 2021 – 2022 was identified retrospectively using OPSC-4 codes. Medical records 132 retrieved, data points extracted. Results We 103 female 29 patients. The cohort had a younger age distribution (median 52y) compared male (59y). 42 underwent total thyroidectomy, 89 hemithyroidectomy 1 lobectomy. 21% specimen reported be malignant, which 74% papillary cell carcinoma, 15% follicular carcinoma 11% Hurthle carcinoma. Our malignancy Thy1a/c category higher (5%) figures risk by RCPath (4%), whilst rest lower than (Thy2 = 0%, Thy3a 13%, Thy3f 19%, Thy4 50%, Thy5 94%). 100% Thy3, diagnostic hemithyroidectomy. 15 did not have documentation U prior thyroidectomy. There 84% compliance repeat FNAC requirements surgery 97% MDT discussion. Conclusions Current BAETS guideline states that nodules benign appearances (U1 U2) do need followed-up, however we found these Costs, risks benefits regularly undertaking fine needle aspiration must considered.
